Job Description Summary

The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel.

Essential Job Functions/Responsibilities

1. Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.

2. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.

3. Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

4. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

5. Attends case conference meetings with organization personnel to facilitate coordination of care.

6. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.

7. Assists in the screening and interviewing process of new organization personnel and
makes recommendations for employment of individuals. Assists in the orientation of
new organization personnel.

8. Assists Clinical Director in the planning, implementation and evaluation of inservice and continuing education programs.

Job Description Summary

The home health aide/CNA is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide /CNA is responsible for observing patients, reporting these observations and documenting observations and care performed.

The home health aide/CNA will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.

Essential Job Functions/responsibilities

Responsibilities of the home health aide/CNA include, but are not limited to, the following:

1. Providing hands-on personal care including:

Job Limitations

The home health aide will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.

Job Description Summary

The Infusion Therapy is responsible to oversee home infusion therapy services and develop professional relationships with physicians, pharmacists, home infusion nurses and health care facility discharge planners

Essential Job Functions/Responsibilities

  1. Collaborates with home health team to identify potential patients for home infusion therapy.
  2. Performs initial assessment to determine eligibility for home infusion therapy.
  3. Assigns clinician to make home visit within 24 hours of referral to assess the home environment and initiate services.
  4. Coordinates patient care between pharmacist, clinicians, and physician regarding initial drug therapy, any drug therapy problems, and changes in administration.
  5. Facilitates communication between patient, family, and caregiver, clinicians, pharmacist, physician, and delivery personnel regarding the supplying, resupplying, and pick-up delivery schedule.
  6. Makes visits to organization patients who have been hospitalized to keep abreast of condition and impending discharge.
  7. Keeps physicians, hospital discharge planners, and organization personnel, as appropriate, informed of home care patient progress following hospital discharge.
  8. Responds to billing department needs and confirms that appropriate documentation is available, regarding home infusion therapy.
  9. Performs direct patient care, as needed, in an efficient, effective, and clinically correct manner.
  10. Conducts inservice programs regarding home infusion therapy services and specific infusion skills for nurses.
  11. Provides portions of orientation and training for new organization personnel regarding home infusion therapy services.
  12. Serves as a resource for nurses and pharmacists regarding information about home infusion services.
  13. Provides input to assist the Clinical Director and Clinical Supervisors in the evaluation of organization personnel performance.

Job Description Summary

The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement.

Essential Job Functions/responsibilities

1. Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.

2. For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.

3. Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Organization professionals.

4. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.

5. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.

6. May train patient in the use of prosthetic and/or orthotic devices.

7. Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with Organization policy.

8. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.

9. Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.

1. Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but will not be limited to:

A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

B. Assesses for muscle strength, mobility, gait, ROM—potential for rehab.

C. Directing physical therapy treatment.

D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

2. Job Description Summary

The Office Manager is responsible for coordinating all office activities including: medical records, data entry, maintaining patient records, preparing for all meetings, coordinating all telephone calls and maintaining sufficient office supplies. The Office Manager is responsible to the Administrator and the Program Director and assists with: direct patient expenditures coordination, employee time sheets, personnel records and billing liaison.

1. Maintains confidentiality of patient information.

2. Maintains accurate up to date medical records in accordance with regulations of Medicare and the Policy and Procedure Manual.

Job Description Summary

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Essential Job Functions/responsibilities

Patient Care

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).

2. Regularly re-evaluates patient nursing needs.

3. Initiates the plan of care and makes necessary revisions as patient status and needs change.

4. Uses health assessment data to determine nursing diagnosis.

5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

7. Counsels the patient and family in meeting nursing and related needs.

8. Provides health care instructions to the patient as appropriate per assessment and plan of care.

9. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Communication

1. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.

The medical social worker is responsible for the implementation of standards of care for medical social work services.

Essential Job Functions/responsibilities

1. Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.

2. Carries out social evaluations and plans intervention based on evaluation findings.

3. Maintains clinical records on all patients referred to social work.

4. Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.

5. Provides information to patients or families/caregivers and community agencies.

6. Serves as liaison between patients or families/caregivers and community agencies.

Job Description Summary

The speech-language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

Essential Job Functions/responsibilities

1. Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

2. Identifies functional speech defects and establishes a plan of care to improve patient’s function.

3. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

4. Recommends communication devices/aids as indicated.

5. Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

6. As appropriate, participates in case conferences, inservices and other meetings required to ensure coordinated and comprehensive care.

7. Prepares and submits notes per organization policy.

8. Maintains clinical competency in speech language pathology practice and theory.

9. Identifies patient and family/caregiver needs for other services and refers as appropriate.

10. Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.

11. May supervise home health aide in procedures directly related to the speech-language therapy plan of care.

3. Successfully completed an approved national examination in SLP

2. A minimum of one year’s clinical experience in Speech-Language Pathology.
